Kendra Cain, owner of Royals MedSpa in Watford City appeared before a Northwest Judicial District Judge Monday, Jan. 26, following her arrest on Sunday, Jan. 25.

Cain has been officially charged with three Class C Felony charges for Reckless Endangerment-Extreme Indifference, Unauthorized Use of Personal Identifying Information-Obtain Credit, and Deceptive Writings-Issue $1,000-$10,000. She is also facing a Class B Misdemeanor charge for Practice as a Nurse Without Proper certification.

On Monday, Judge Schmidt set Cain’s bond at a $5,000 unsecured appearance and scheduled her preliminary hearing/and or arraignment for March 26, 2026.

New Details of Case
According to the affidavit filed with the Northwest Judicial District, the Watford City Police Department (WCPD) began investigating a report alleging the unauthorized practice of nursing and criminal impersonation at Royals MedSpa on Jan. 22.

During the investigation, law enforcement conducted  licensure verification through the North Dakota  and National nursing licensure databases, which revealed no record of Cain holding any type of nursing license.

Further investigation revealed one employee of Royal MedSpa as a licensed registered nurse, identified as RN Widvey.

Law enforcement conducted an interview with RN Widvey who stated she was the only licensed RN at the facility. She reported that Cain was utilizing her license credentials without authorization to schedule patients, order medications and maintain clinic operations when RN Widvey was not present.

Widvey also informed law enforcement that Cain would place patients on Widvey’s schedule despite her not being on site or providing care. She further reported that Cain repeatedly represented herself as a registered nurse by adding “RN” to her name on Facebook posts, emails and other communications. Widvey provided screenshots of an email and Facebook posts, along with an audio recording of Cain identifying herself as an RN.

Law enforcement then visited with the medical director for Royals MedSpa, who stated that  upon having the allegations of Cain brought to her attention, she requested that Cain provide proof of nursing licensure.

She stated that Cain was not able to provide documentation and later admitted she did not possess an active nursing license. Cain told the director that she previously  attended a nursing program, but did not complete licensure.

It was through the interviews conducted during the investigation that law enforcement determined Cain was knowingly misrepresenting herself as a licensed nurse and engaging in medical activities without lawful authority.

This is an ongoing case. The Farmer continues to investigate information surrounding the charges and will report more details as they are confirmed.
