Arrest made following abrupt shutdown of Royals MedSpa

On Sunday, Jan. 25, Royals Medspa Owner Kendra Cain was arrested and is now facing a multitude of charges: One Class A Misdemeanor of Deceptive Writings; One Class B Misdemeanor for Practicing as a Nurse Without Proper Certification; and Two Class C Felony charges for Unauthorized Use of Personal Identifying Information and Reckless Endangerment-Extreme Indifference.


Cain is currently being detained at the McKenzie County Correctional Facility and is scheduled to appear in court today.


The Farmer first reported information following several social media posts made Saturday, Jan. 24, The Farmer received multiple inquiries from community members seeking answers as to the looming question “What happened to Royals Medspa?’


Sources told The Farmer, that the Main Street business, which opened September 2025, was shut down on Thursday, Jan. 22, following reports that the owner, identified as Kendra Cain, has been operating her business without a nursing license.


Cain has promoted Royals Medspa as a “luxury medical spa that combines precision medical aesthetics with proactive wellness - offering advanced laser tattoo & hair removal, skin resurfacing, massage therapy, IV therapy, facials, peptides, neurotoxin, filler, paramedical tattooing, and personalized wellness guidance,” according to www.beautyhairnailsalons.com


The Watford City Police Department confirmed Saturday, that it is investigating complaints related to Royals MedSpa alleged unlawful business practices. 


The WCPD advised that individuals with specific concerns regarding services that they may have received from Royal MedSpa are encouraged to contact WCPD Dispatch at 701-444-2400. Callers will be directed to Detective Bostic or Officer Hunt.


As of Monday, Jan. 26, the WCPD provided the following information to The Farmer.


On January 22, 2026, the Watford City Police Department (WCPD) received information alleging that the owner of Royal Med Spa, located at 361 Main St. N., Kendra Cain, had been representing herself as a Registered Nurse (RN) and performing medical procedures without lawful authority.


Officers and detectives conducted a multi-day investigation which determined that Cain had been identifying herself to the public and to patients as a Registered Nurse. Investigators confirmed through state and national nursing and medical provider databases that Cain does not hold any nursing or medical licenses. The investigation further revealed that Cain allegedly used a staff member’s medical license to schedule patients, prescribe medications, and perform medical procedures.


During the evening hours of January 22, 2026, WCPD officers executed a search warrant at Royal Med Spa. During the search, officers located a patient treatment area containing a used IV bag, tubing, syringe, needle, bandage, and an open sharps container. Officers also observed what appeared to be fresh blood on the floor beneath the IV infusion area. Cain admitted to officers that she had administered medical treatment to a patient using the items located in the treatment area.


On January 23, 2026, officers spoke with the patient who received the treatment. The victim, who is a registered nurse at a local medical facility, reported that she had to coach Cain through the IV administration process. The victim further stated that the tourniquet used during the IV placement was left on longer than appropriate, causing her hand to turn blue and purple. The victim also reported observing approximately three to four inches of air traveling through the IV tubing into her vein, which she stated could potentially result in an air embolism, a condition that can cause serious injury or death.
